Revise documentation of node/structuring commands
* doc/texinfo.texi (Tree Structuring): Remove example of
@node/@chapter completely.  This may have been next to the hierarchical
chart to explain node pointers but the example doesn't use them.
(Chapter Structuring): Put basic examples of @chapter or @node/@chapter
here.
