   * doc/texinfo.tex (\ignorenode): Stop ignoring node at top-level section commands: @chapter, @centerchap, @unnumbered, or @appendix.  This allows a lone @chapter to appear immediately after the Top node without being elided. (\chapter, \appendix\, \unnumbered, \centerchap): Remove \outer from definition as this makes it hard to manipulate these sequences using TeX macros.
