Use <hr> in index formatting
* tta/C/convert/format_html.c (html_convert_printindex_command),
* tta/perl/Texinfo/Convert/HTML.pm (_convert_printindex_command):
Use <hr> to separate sections of index.
* tta/data/default_css_element_class_styles.csv
(th.index-letter-header-printindex):
Remove "border-width: thin; border-top-style: solid".

This is a partial reversion of change to index formatting made
on 2024-12-24.
