   urlapi: return NULL from curl_url_dup() when given a NULL handle
   Previously, passing a NULL pointer to curl_url_dup() caused a NULL
   pointer dereference crash on in->scheme inside the DUP() macro.
   
   Return NULL immediately when !in, matching curl_easy_duphandle() and
   preventing unexpected crashes when duplicating uninitialized or failed
   handles.
   
   Also verify this behavior in test 1560 and update documentation.

   connection pool: use tbl and bsets
   Handle concurrent modifications to the connection pool, e.g. removing
   connections while iterating the pool/a bundle, by replacing the llists
   with a uint32_tbl and uint32_(sp)bsets.
   
   - Introduce a `uint32_t cpid` connection pool identifier assigned
     to connections when added to a pool.
   - Keep track if idle connections via their `cpid`
   - Track `data->state.last_cpid` for fast lookups of the last
     connection used by a transfer
   - Add reference counter to `cpool_bundle`s, so a bundle does
     not get destroyed while iterating it and the last connection
     gets removed from the bundle
