    http2: use stream in userdata
Change nghttp2 stream userdata from the transfer to the `h2_stream_ctx`
and lookup the transfer at the multi's table. This replaces hash lookups
with table lookups which are faster.

When the connection is closed, a websocket client writer that
sees the EOS in the middle of a frame errors out.

    http: reject empty header names
A custom proxy header can have a non-empty name that becomes empty after
trimming. An HTTP/1 header line can also start with a colon. Reject
these inputs before they reach Curl_dynhds_add(), which requires a
non-empty name.

Keep custom proxy headers consistent with existing behavior by ignoring
the malformed entry. Return CURLE_BAD_FUNCTION_ARGUMENT from the HTTP/1
parser.

Add unit and proxy CONNECT regression coverage.

    connect: do not busy-loop in blocking connect without a timeout
- During blocking connect treat a transfer/connection timeout of 0 as
  timeout of 1000 for polling.

Prior to this change if no timeout was set then 0 was passed as the
timeout time to poll which caused a busy loop since it returned
immediately.

    multi: is_finished() fix
When the progress callback errors on a not comppleted transfer, the
transfer state entered COMPLETED right away and not DONE. This led
to cleanup actions on the transfer/connection to not run.

Only go to MSTATE_DONE in this case.
