    tool_ipfs: keep the path percent-encoded when rewriting to the gateway
The IPFS/IPNS rewrite fetched the user path (and the CID and gateway
path) with CURLU_URLDECODE and then re-encoded the assembled path with
CURLU_URLENCODE. The decode turned an encoded slash (%2f) into a real
separator that the URL parser's earlier dot-segment cleanup never saw,
so a ".." could climb back out of the /<protocol>/<cid> namespace when
the final URL was parsed. A double-encoded form (%252e%252f) slipped
through the same way.

Build the new URL from the still-encoded parts instead: get the CID, the
gateway path and the user path without decoding, and set the combined
path without re-encoding. A percent-encoded slash then stays encoded, so
no new separators appear and the later normalization has nothing extra
to remove. A literal "../" in the user path is still collapsed by the
parser when the ipfs:// URL is first parsed, so it resolves within the
namespace as before.

Add tests 1691 (ipfs) and 1692 (ipns) that send an encoded-slash dotdot
path through a gateway with a base path and verify the request stays
under /<gateway>/ipfs|ipns/<cid>/. Both fail on the previous code with a
request for /admin/config.

    cw-pause: fix O(N^2) list traversal in cw_pause_flush
Switch the pause buffer list from prepend-at-head to append-at-tail and
add a tail pointer to cw_pause_ctx. cw_pause_flush can now consume nodes
from the head in O(1) per node instead of re-scanning from the head on
every outer iteration.

    u32_ptrset: a set of uint32_t + void* values
This replaces uint32_hash as a leaner and faster implementation.
It uses the same internal logic as u8_strset for hashing ids.

Use in http2 and QUIC implementations to connect `data->mid` to
the `stream` struct of the protocol implementation.

Remove uint-hashset.[ch] and its unit test.

    http: abide to close-rules when transfer-encoding off
There is no other way to know when the transfer ends when there is a
transfer encoding set but not chunked - even if transfere-encoding is
disabled. Tweak test 319 to verify this (by setting a wrong
Content-Length).

Add test 2918 to make sure it works the same also when the
Content-Length and Transfer-Encoding headers arrive in the other order

Fixed test 1494, it needs to close after "Transfer-Encoding: identity".
